State health club statutes can constrain assignment, refunds, and cancellation rights in ways the MCA paperwork ignores. The owner should identify the governing law without assuming that a form drafted for another state answers anything, and a billing report that looks identical in two jurisdictions can support two different collection plans, and I am less certain than this section sounds about how a court would choose between them.

Billed and unbilled amounts, refundable deposits, and whatever the billing provider withholds under its own contract each change the collection picture. The review should address each category from the documents rather than from the totals on the screen, which present an extremely confident face (the screen always looks better than the bank).
